Facial Rejuvenation with Rhinoplasty
My Approach
Beauty is always found in the details. This beautiful 72 year old woman presented after prior facial surgery performed elsewhere, including a facelift, fat grafting, and upper eyelid surgery with excessive fat removal. Over time this left her with hollow upper orbits, an elongated upper lip with minimal tooth show, and a plunging, aged nasal tip. In addition, the normal signs of aging around the eyes, mouth, face, and neck had progressed. When multiple small imbalances coexist, no single procedure can solve the problem. The face must be treated as a complete unit. She underwent a carefully planned combination of procedures including a unilateral right direct brow lift, minimal fat grafting, conservative hyaluronic acid filler, full face CO₂ laser resurfacing, secondary deep plane SMAS face and neck lift, endonasal upper lip lift, Valentine corner of mouth lift, revision upper and lower blepharoplasty, and open rhinoplasty. At only two months post operative, the improvements are already evident yet subtle. Her eyes are no longer hollow or deep set, the upper lip is shorter with improved tooth show, the nasal tip is refined and supported, and the neck contour is smooth. Most importantly, she looks refreshed and natural, without any suggestion of having had surgery. Small, precise adjustments across multiple areas create harmony. When patients look odd after cosmetic surgery, something is out of balance. Restoring prior unfavorable results requires far more planning, judgment, and restraint than primary surgery.

The Healing Process
Early swelling and tightness are expected during the first few weeks. Most bruising resolves within two to three weeks, while subtle swelling continues to improve over several months. Laser resurfacing redness gradually fades, and incisions soften and mature over time. By two months, patients look socially presentable, with continued refinement for up to a year. Secondary or revision cases may take slightly longer to fully settle, but the improvements become more natural as healing progresses. Patients are advised to follow postoperative instructions closely, including sun protection, gentle skincare, and activity restrictions. Regular follow-up visits allow healing to be monitored and concerns addressed promptly. Patience is essential, as final outcomes reflect gradual tissue remodeling rather than immediate change over time with consistent care and realistic expectations throughout recovery for optimal results.
FAQs

It shortens a long upper lip and increases tooth show, restoring youthful proportions and improving the smile.
Most patients resume light activity within two weeks and feel comfortable socially by four to six weeks, with ongoing improvements for several months.
Yes. Secondary deep plane techniques can refine contour, reposition deeper structures, and correct prior shortcomings.
Overcorrection leads to an unnatural look. Conservative, precise adjustments create the most believable and lasting results.
Patients with multiple signs of aging or prior surgery who desire natural, balanced improvement rather than dramatic change benefit most from this approach.
