This 75 years young, thin female presented after a facelift performed a decade earlier with an unsatisfactory outcome and progressive, accelerated aging of the face, neck, mouth, and eyes. A facelift alone is rarely enough in patients with global facial aging. Treating only the skin or neck often leaves the eyes, lips, and midface discordant. True rejuvenation requires addressing the entire face as one harmonious unit. She underwent comprehensive facial rejuvenation including an extended deep plane vertical face and neck lift, soft tissue augmentation, lip lift, corner of mouth lift, upper eyelid blepharoplasty with correction of hollow orbits, and precise neurotoxin placement. Early photos at two days show the immediate structural improvement, while her eight week result demonstrates a softer, more natural appearance as swelling resolves. Her neck contour is smoother, the midface is lifted, the mouth corners are restored, and the eyes appear fuller and more youthful without hollowness.
