My Approach
True body contouring is about structure, planning, and execution — not gadgets. This middle-aged female patient was in excellent physical condition with a classic gynecoid shape, but presented with buttock ptosis, skin laxity, and cellulite related to weight loss and natural aging. Importantly, the degree of ptosis varied relative to the length of the gluteal crease — a detail that significantly impacts surgical planning. Rather than relying on fat grafting, implants, or energy-based devices, we addressed the root cause: excess skin, tissue descent, and loss of structural support. A circumferential (360-degree) body lift was performed with autologous buttock augmentation, using her own tissue to restore projection and shape. Vertically, 16 cm of redundant skin was excised over the buttocks, allowing true elevation and reshaping of the gluteal region. Special attention was paid to scar placement. A non-traditional low scar was positioned within the gluteal crease, keeping it discreet while maximizing lift and contour. This approach improved not only buttock position, but also skin texture, laxity, and the appearance of cellulite. In my opinion, technologies such as J-Plasma, BodyTite, and radiofrequency devices do not effectively correct this type of problem. Skin and tissue descent require surgical repositioning — not heat. Meticulous pre-operative analysis followed by precise execution is what creates durable, natural results.

The Healing Process
Recovery after a 360 body lift with autologous buttock augmentation requires patience but is well tolerated by most patients. During the first 1–2 weeks, swelling, tightness, and soreness are expected. Patients are encouraged to walk early but must avoid prolonged sitting or pressure on the buttocks initially. Compression garments are worn to support healing and reduce swelling. By four weeks, much of the swelling has improved and early contour changes are clearly visible, though tissues are still settling. Incisions appear pink early on and gradually mature over several months. Most patients return to light activities within 2–3 weeks and resume more strenuous exercise around 6–8 weeks, depending on healing progress. Final results continue to refine over 3–6 months.
