This beautiful 61 year old presented with visible aging throughout the lower face and neck, including skin laxity, soft tissue descent, and loss of jawline definition. She also had naturally prominent submandibular glands, which can sometimes create fullness beneath the jaw. In situations like this, the key is restraint and thoughtful planning. Removing or aggressively altering normal anatomy often leads to unnatural results. Rather than chasing an overly tight neck or attempting to remove the glands, we elected to preserve them and focus on repositioning the deeper facial structures. A deep plane facelift was performed to elevate and restore the descended tissues at their foundational level. By lifting the muscle and ligament layers together instead of simply tightening the skin, we achieved smoother contours, longer-lasting support, and a more natural transition from the jawline into the neck. To correct mild brow asymmetry, a unilateral right brow lift was added for balance. Small, targeted adjustments like this often make a significant difference in overall harmony without making the face look “done.” It is extremely important not to be overly aggressive in the neck, especially in patients with larger submandibular glands. Over-tightening can create a hollow or operated look and may become more obvious with future weight loss. A conservative, anatomical approach allows the neck to look elegant and natural both now and years from now. At six months, her result shows a refined jawline, improved neck definition, and beautifully concealed scars hidden around the ear. She looks refreshed and youthful, not surgically altered.

The Healing Process
Recovery after a deep plane facelift is typically very manageable. During the first week, swelling, tightness, and mild bruising are expected, particularly around the cheeks and neck. Most patients feel comfortable resting at home during this time and return to light daily activities within 7 to 10 days. By two to three weeks, the majority of visible swelling and discoloration has resolved, and patients usually feel confident being out socially.
