My Approach

After significant weight loss, the challenge is rarely excess fat—it’s excess skin and tissue laxity. The breasts, lateral chest, back, and abdomen can all descend together, creating folds and bunching that don’t respond to exercise. Treating only one area in isolation often leads to incomplete or unbalanced results. My philosophy is to evaluate the torso as a whole and design a comprehensive plan that restores proportion circumferentially rather than focusing only on the front of the abdomen. For this patient, concerns included severe breast ptosis with implants, lateral torso and axillary descent, and 360-degree abdominal laxity. To address these issues safely and effectively, we used a staged approach. Procedures included implant removal followed by a secondary breast lift, axilloplasty to correct lateral rolls and underarm fullness, reverse abdominoplasty with an upper back lift to tighten the upper torso, and an oblique flankplasty to contour the waist and flanks without placing visible incisions on the front of the abdomen. Careful incision placement respected natural skin tension lines and preserved vascularity, helping optimize both healing and scar quality. The goal wasn’t simply tightening—it was creating smooth, clean contours that look natural in fitted clothing and swimwear.

The Healing Process

Recovery depends on the combination of procedures performed and whether surgery is staged. Most patients experience swelling, tightness, and temporary bruising for the first few weeks. Light walking begins immediately, while more strenuous activity is typically resumed after four to six weeks. Incisions may appear pink or red early on and gradually fade over several months. As swelling resolves, the torso becomes progressively smoother and more defined, with final contour visible over three to six months.By week three, most patients feel comfortable resuming normal activities. Facelift results settle within a few months, while the nose continues to refine for up to a year.
