My Approach

This beautiful, athletic young woman presented after multiple prior breast surgeries performed elsewhere. Over the years she had undergone numerous above-the-muscle saline and silicone implant placements that resulted in persistent pain, malposition, asymmetry, traction rippling, over-dissected pockets, capsular contracture, and progressive ptosis. There had even been unsuccessful attempts to convert the implants under the muscle along with the imprudent use of scaffold materials. Situations like this are rarely caused by one single issue. They are typically the cumulative result of poor planning, excessive dissection, and a lack of respect for anatomy and tissue integrity. The goal in complex revision surgery is not simply to “change implants,” but to rebuild the foundation from the inside out. Her reconstruction was performed through the areola to minimize additional scarring. The large 605 cc silicone implants were removed completely. The pockets were corrected and tightened, and we converted to a true under-the-muscle position for better coverage and long-term stability. Torn muscle on the right side was repaired, and internal support was reinforced with acellular dermal matrix grafts to restore structure and control implant position. She then received moderate-profile 520 cc inflatable saline implants for improved symmetry and a lighter, more controllable implant choice. A bilateral circumareolar lift refined nipple position and corrected the residual ptosis. At just seven weeks, her breasts already appear more balanced, softer, and anatomically positioned. Most importantly, the discomfort and distortion from the previous surgeries have been addressed. Revision breast surgery demands judgment, craftsmanship, and restraint. When anatomy is respected and the plan is executed meticulously, even the most challenging cases can be restored to a natural, elegant result.

The Healing Process

Early tightness and swelling are expected during the first one to two weeks. Most patients return to light daily activity within several days and routine tasks within a week. Because this is reconstructive revision surgery, swelling may take slightly longer to settle compared to primary augmentation. Implant position continues to refine over several months as tissues relax and scars mature. Incisions around the areola typically heal very well and blend naturally with the surrounding skin. Final softness and contour improvements continue for three to six months, with scars fading over the course of a year. Supportive garments are worn during early healing to protect the reconstruction and maintain optimal implant positioning.
